Report Suspected Wrongdoing
Use the form below to report any suspected waste, fraud, abuse or unethical activity that you have witnessed or experienced in San Francisco. Reports may concern individuals, businesses, organizations or public entities.

How This Process Works
- Confidential Review: The campaign reviews all submissions confidentially
- Anonymous Submissions Welcome: You may submit reports anonymously
- Appropriate Referral: The campaign forwards credible reports to relevant government entities with jurisdiction
- Detailed Information Helps: Provide as much detail as possible to support thorough investigation
What to Report
Examples of waste, fraud and abuse include:
- Misuse of public funds
- Conflicts of interest
- Procurement irregularities
- False claims or billing
- Employee misconduct
- Contract fraud
- Regulatory violations
- Bribery or kickbacks
- Nepotism or favoritism
- Fraudulent grants or permits
What to Report
Examples of waste, fraud and abuse include:
- What happened: Describe the suspected wrongdoing
- Who was involved: Names, titles and organizations
- When it occurred: Dates or timeframe
- Where it took place: Location or department
- Evidence: Documents, communications or other supporting materials
- Witnesses: Anyone else who can corroborate your report
